Residency Affidavits

Beginning October 4, 2021, Safe & Secure Schools will only accept affidavits electronically. All affidavits and supporting documentation may be submitted via email to or faxed to 281-284-9903.

2021-2022 Affidavit Forms

Please be advised all affidavits are:

  • One per student
  • Required to be notarized prior to arriving at Safe and Secure Schools
  • Required to show the supporting documents as listed on each packet
  • Allowed to turn in a tax statement if there is no lease or mortgage statement

Only one student per original affidavit. No copies or notary services are available at the Safe and Secure Schools. The Affidavit and attachments will be reviewed while you are on location and upon approval the affidavit, Safe & Secure Schools will submit the information to the according campus.

Safe and Secure Schools


Students currently attending schools in the Clear Creek Independent School District under a residency affidavit must have their residency verified annually to avoid withdrawal from the Clear Creek Independent School District. The students affected are those currently enrolled on an affidavit.

If you no longer require a residency affidavit, please deliver copies of the required proof of residency documents (mortgage, lease and utility bill in your name) to the school to correct their records.
