New Student Registration
for the 2006-07 School Year
July 31st – August 9th
8AM to Noon
Evening Registration – August 1st 5:00 to 7:00PM
You will need to bring:
1. Certified Birth Certificate (Kinder students must be 5 years of age by September 1st.)
2. Social Security Card
3. Immunization Records
Minimum of 4 DTP's. Last one given after 4th birthday
Minimum of 3 Polio's Last one given after 4th birthday
MMR: 2 First one given after 1st birthday.
Second one at least 30 days apart
Hib: Primary series and booster OR one dose
on or after 15 months of age
Varicella: Minimum of 1 vaccine given after 1st birthday or verification of disease
Hepatitis B Series of 3 doses
4. 2 Proofs of Residency* (Mortgage coupon booklet or lease
agreement and a current utility bill.)
5. Parent/Guardian Driver’s License
6. Student Last Report Card
7. Please be prepared to complete our Registration Packet.
Please note that up to date immunizations and proofs of residency are required before we can place your child in a class.
*Acceptable Proof of Residency
Two (2) Proofs Are Required For New Enrollment
One
(1) Proof Is Required For Continuing Enrollment
One of the following:
· Rental Contract on Residence
· Lease Contract on Residence
· Mortgage Statement
· Deed on Residence
· Payment of Interest Statement (IRS Form)
AND
One of the following utility bills:
· Electricity
· Gas
· Water/Sewer
· Telephone
Cable bills and cellular telephone bills are not acceptable.
