Student Registration

will not be able to register your child without all of the documents
and proofs listed below.)
In order to register
your child, a parent or guardian must present the following items:

Parent/Guardian driver's
license

The child's official state
birth certificate

The child's social security
card

The Family's proof of
residence (signed lease agreement or current utility bill in the family's
name)

Immunization records:
DPT - 4 doses, including one dose after 4th birthday
Polio - 3 doses, including one dose after 4th birthday
MMR - 1 dose on or after 1st birthday
Measles - 2nd dose, if born after 9/2/91
Hepatitis B - 3 doses if born after 9/2/94
Varicella - 1 dose if born after 9/2/94 or history of Chicken Pox
