Students attending schools in the Clear Creek Independent School District under a residency waiver must have their affidavits renewed no later than January 15, 2008 to avoid withdrawal from the Clear Creek Independent School District. The students affected are those living with a guardian or being cared for by a grandparent after school and would otherwise be zoned to a different school district.

Completed form must be notarized. No other documents are required if you completed an affidavit in the FALL 2007.
Upon completion, please bring duplicate PHOTO COPIES of the affidavit(s) and required documents to the Department of Safe & Secure Schools at 2425 E. Main Street, League City, Texas. NOTE: No photo coping or notarizing of documents will be made on location. The paperwork will be reviewed while you are on location and upon approval, you will be given a copy to deliver to the campus for their records. Please DO NOT bring the un-approved form to the student’s school. The affidavit must be processed through the Department of Safe and Secure Schools first.
If you no longer require a Residency Affidavit, please deliver copies of the required proof of residency documents (same as above) to the school to correct their records.
