If you are a HOMEOWNER:
- Current mortgage payment coupon, IRS Interest Form 1098, or Deed of Trust
If you are LEASING:
- Current lease agreement of Resident with the names of parent/Children as occupants on the lease agreement
ALL APPLICANTS: Please provide both of the following:
- Current utility bill (phone or cable bills not acceptable) AND
- Texas ID/Drivers License of both the Resident and Parent completing the affidavit. The address on the ID/Drivers License should have the address residing at as listed on the affidavit.
Upon completion, please bring 2 PHOTO COPIES of the affidavit(s) and required documents to the Department of Safe & Secure Schools at 2425 E. Main Street, League City, Texas. NOTE: No photo coping or notarizing of documents will be made on location. The paperwork will be reviewed while you are on location and upon approval, you will be given a copy to deliver to the campus for their records. Please DO NOT bring the un-approved form to the student’s school. The affidavit must be processed through the Department of Safe and Secure Schools first.
