* If your event is less than 15 working days away, your request WILL NOT be considered. Any submission that is less than 15 days from the date of submission will be automatically thrown out of the system.

The information requested in this form must be submitted to complete your request. If you do not provide all information requested we can not complete your rental. By submitting this information and requesting a facility rental you acknowledge:

The representative and the organization which he/she represents hereby agree and undertake to save and hold blameless the school district from any and all claims for damages, personal or otherwise, that may arise out of the use of the property, whether by a member of his organization of by other persons using or enjoying said property, and without requard to whether the damage, personal or otherwise, is brought about or caused by negligence, whether on the part of the representative, organization, the school district, or all three.
ADDITIONAL DOCUMENTATION:
You must provide a copy of a Certificate of Insurance naming CCISD as an additional insured in the amount of one million dollars.

If you are claiming non-profit status we will need a copy of your 501(c)(3).

Indicate in the subject line the name of your organization and the date your rental request.

Schools will not be available for use on holidays of teacher/staff development days, unless special arrangements have been made and approved.

Payment for event(s) must be paid in full upon execution of the rental contract.
ACKNOWLEDGEMENT:
- At no time shall the facilities be used without the presence of a representative of the school district.
- School property will not be used where the purpose is in conflict with the aims and objectives of the public school system; nor shall property be used for un-American, sacrilegious, or controversial purposes. All national and state laws, local ordinances, and the rule of the police and fire departments and school district (no smoking, alcoholic beverages, or ballon releases, etc.) shall be obeyed.
- The representative and organization will be responsible for and agree to pay for damages done, exclusive of ordinary wear and tear.
- SUBMISSION CONSTITUES THAT THE SUBMITTER HAS READ AND UNDERSTANDS CCISD's FACILITY RENTAL POLICY.
