Student Insurance
Parents are encouraged to ensure their student have adequate insurance to cover treatments related to injuries. The District offers a voluntary student accident insurance to help defray the costs of medical expenses. Click here to download information pertaining to the Student Accident Insurance. Enrollment and Permission Forms
Physical Form - Required Form for All Athletic Participation
This form MUST be filled out COMPLETELY, signed as indicated, and on file with the athletic trainer BEFORE the student will be allowed to participate in any class period rehearsal, tryout, practice session, performance or camp for band, color guard or drill team.
