Kindergarten:
The 2017-2018
Affidavits will be accepted starting April 24, Monday-Thursday, 8:30 a.m. - 4:30 p.m.
1st-12th grade:
The 2017-2018 Affidavits will be accepted beginning August 1, 2017, 8:30 a.m. - 4:30 p.m.
The following packets are for the 2017-2018 school year only. If you are in need of an affidavit for the current school year, contact the
Safe Schools Division for assistance 281-284-0063.
Packet A - Parent's/Guardian's Assurance of Bona Fide Residence
Packet A - Garantía de buena fe de padres y representantes para la residencia
This form is used if the Parent does not have a lease or mortgage statement in their name and resides with someone in the Clear Creek ISD attendance zone. The attachments required along with the completed Affidavit are as follows:
- Parent’s Government Issued Photo I.D.
- Student’s Birth Cert. or other acceptable proof of identity
- Resident’s Government Issued Photo I.D.
- Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
- Current Lease or Mortgage Statement (Tax statement may be substituted)
- Only one student per original affidavit.
Packet B - Grandparent’s After School Care Affidavit
Packet B - Garantía del cuidado después de la escuela por los ABUELOS para los estudiantes que no son residentes
The Grandparent Affidavit may be used only if the Parent lives outside the CCISD attendance zone and the Grandparent live within the CCISD attendance zone. The attachments required along with the completed Affidavit are as follows:
- Parent’s Government Issued Photo I.D.
- Parent's Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
- Student’s Birth Cert. or other acceptable proof of identity
- Grandparent's Government Issued Photo I.D.
- Grandparent's Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
- Grandparent's Current Lease or Mortgage Statement (Tax statement may be substituted)
- Campus may request a copy of the Parent’s Lease or Mortgage statement to verify they reside outside the attendance zone.
- Only one student per original affidavit.
Packet D - Guardianship Notification
Packet D - Notificación de la tutela
Guardianship Affidavit notification is used if the parent is signing over their educational rights to a person that is not a relation to the child (ex. Family friend, neighbor).
*This Affidavit does not take away from the parental rights in any way, but only grants educational rights to the guardian and gives the campus permission to discuss the student with the guardian. The attachments required along with the completed Affidavit are as follows:
- Parent’s Government Issued Photo I.D.
- Student’s Birth Cert. or other acceptable proof of identity
- Guardian’s Government Issued Photo I.D.
- Guardian's Current Utility Bill (Gas, Water or Electric – Disconnect Notices will not be accepted)
- Guardian's Current Lease or Mortgage Statement (Tax statement may be substituted)
- Only one student per original affidavit.
The completed original affidavit including all attachments and a copy of
the original affidavit including all attachments must be returned to
the Safe and Secure Schools Department located in the
Education
Support Center at 2425 East Main Street in League City, Texas for
approval.
Please Note: Only
one student per original affidavit. No copies or notary services are
available at the Safe and Secure Schools Department located in the
Education Support Center. The Affidavit and attachments will be
reviewed while you are on location and upon approval the affidavit will
have an embossed stamp from the Safe and Secure Schools Department
before affidavit is returned to the campus by the Parent or Guardian.

Renewal Verification Affidavits
We will no longer be accepting renewal verification affidavits beginning with the 2017-2018 school year. Students currently attending schools in the Clear Creek
Independent School District under a residency affidavit must have their
residency verified annually to avoid withdrawal from the Clear Creek
Independent School District. The students affected are those currently
enrolled on an affidavit.
NOTE: No photo copying or notarizing of documents will be made on location.
If you no longer require a residency affidavit, please deliver copies of
the required proof of residency documents (mortgage, lease and utility
bill in your name) to the school to correct their records.
